CAEHR 2

The department of General practice is part of the collaborative research project CAEHR2 (Cardiovascular Diseases – Enhancing Healthcare through cross-sectoral Routine data integration), which has been running since August 2025 under the project management of the Department of Medical Informatics at the University Medical Centre Göttingen (UMG). 

Project objectives

The aim of CAEHR2 is to link health data across sectors throughout the entire treatment pathway - from prehabilitation through hospitalisation and rehabilitation to outpatient aftercare - with a focus on patients with cardiovascular diseases.

We are involved in the ‘Patient Journey’ use case: here, the aim is to support patients in the long term, both before and after a cardiovascular event. Digital tools - including a secure portal system for exchanging health information and an app for self-assessment of physical fitness - are intended to improve communication between institutions and actively involve patients in their own care.

CAEHR2 is part of the DigiHub initiative and is closely linked to the Medical Informatics Initiative (MII) and the University Medicine Network (NUM).
